Floating Support Services (Housing Related Support Services) - AWARD

Telford & Wrekin Council are commissioning a floating housing related support service in 2 contract Lots:

• oLot 1: A young people's and working age (aged 16-54 years) floating housing related support service for people across all tenures (excluding people living in supported housing).
• oLot 2: A floating housing related support service for older people (aged 55+) living in all housing tenures (excluding extra care housing).

These services are intended to provide:

• oHigh quality personally tailored floating support services to be delivered to those individuals who have an assessed need across Telford and Wrekin.
• oThe floating support services will be of a preventative nature to help individuals to establish and maintain independence, remain in accommodation and lead fulfilling, healthy and independent lives.
• oThe floating support services will contribute to preventing and/or minimising the prevalence of homelessness and to the transformation of Adult Social Care Services helping to shift the demand for services by preventing, delaying and reducing unnecessary or early access to Adult Social Care Services and Health services.

Within each contract Lot, the provider shall provide structured packages of floating support to a caseload of individuals who have an identified need for housing support and to achieve identified outcomes within this specification.

We are inviting responses from qualified and competent providers to deliver one or both lots.
