Home From Hospital

Norfolk County Council wishes to award a contract to a provider to deliver services to support adults on discharge from hospital. The Home from Hospital service will help people re-adjust to living at home again, gain in confidence, and provide short term practical help after hospital discharge. The length of the support is to be no more than 6 weeks. The term of the contract is expected to for 3 years initially with the option to extend for a further 7 years. Referrals to this service will come from one of the three acute hospitals in Norfolk.
