NHS Croydon Clinical Commissioning Group (CCG) is inviting suitably qualified and experienced Providers of Anticoagulation Services to register their interest in this future opportunity. The purpose of this engagement is to inform the contracting approach for the future Integrated Community Anticoagulation service and to gauge the level of market interest.

The CCG is encouraging services to innovate to provide both non-complex and complex care out of hospital in a community setting.

The overall aims of the Integrated Community Anticoagulation service will be to:

Provide a cost effective, standardised, clinically effective and safe community anticoagulation service
Improve quality and clinical outcomes through standardisation of care across an integrated pathway
Enable patients to have equitable access to a range of medications and treatments in a convenient community or domiciliary setting
Provide comprehensive and ongoing education to support medication adherence and patient self-management

The duration of the contract will likely be for 3 years with the option to extend for up to a further 2 years at the CCGs discretion. The CCG intends for the service to commence in April 2018. The indicative annual contract value for this service is approximately £1,300,000.
