Haringey CCG is looking for expressions of interest for the provision of a Self-Management Programme for people with living with type 2 diabetes.
The programme will provide additional support to those people who are not optimally managing their condition. The service will be proactive in engaging hard to reach communities.
The objectives are as follows;
| • | To plan, develop and deliver a type 2 Diabetes specific Self-management Programme for people with type 2 diabetes in Haringey |
| • | To recruit participants to the type 2 Diabetes Self-management Programme to reflect the target demographic |
| • | To build on local assets – people, skills, places – to increase local, Haringey based capability and capacity for future delivery |
| • | Provide opportunities for people from local communities to train as tutors on a Self-management Programmes, thus building local capacity to co-deliver the programme in a range of community languages and across the borough |
| • | An increase in the number of people with type 2 Diabetes who participate in a Diabetes Self-management Programme |
| • | To increase social capital through networks of course alumni, other peer networks and volunteering opportunities |
The service will deliver community development, outreach and capacity building in the local community. Building capacity will include training Haringey residents with type 2 diabetes to deliver the training programme and particularly training residents who have language skills that enable better engagement with hard to reach groups.
The Contract value is in the region of £27,000 to £35,000.
The deadline for Potential Bidders to submit their expressions of interest has been extended to 6pm on 22 June 2015.
