Toftwood Medical Centre APMS

This procurement is to commission GP services to serve the Dereham area of Norfolk, specifically those patients on the current and future GP List for The Toftwood Medical Centre (2 Chapel Lane, Toftwood, Norfolk, NR19 1LD). This service is a GP lead service and must be led by GPs, supported by nurses and any other suitable clinicians throughout core hours as defined in the service specification.

Based on the current contract it is anticipated that the service provision resultant of this procurement will have the following values per year.

Year 1 - £488,834.00 Year 2 - £428,529.00 Year 3 - £368,186.00 Year 4 - £311,568.00 Year 5 - £311,568.00 TOTAL - £1,908,685.00

There is a financial threshold of £1,908,684 - any bids exceeding this contract value will be not be accepted and disqualified accordingly.
