Suffolk County Council is seeking to award a contract for an integrated Healthy Lifestyles Service that will prevent early onset of modifiable conditions, modify lifestyle behaviours and prevent early onset of non-communicable diseases.
Key priorities will be reducing health inequalities, increasing long term health outcomes and improving quality of life for Suffolk residents. Interest is sought from suitably qualified and experienced organisations to provide these services across the whole county of Suffolk.
The Council expects this contract to be flexible and responsive to local needs and to continue to develop, innovate and evaluate high quality Healthy Lifestyle Services. The contract will also improve sustainability of behaviour change by effective partnership working with relevant partner organisations.
The Provider will be expected to support wider objectives prioritised by the Suffolk Health and Wellbeing Board and Suffolk County Council. Future developments for the contract may include but not be limited to the integration of tier 3 weight management services, National Child Measurement Programme (NCMP) and self care (e.g. Expert Patient Programme and patient activations). For NHS Health Checks (Outreach) future developments may include the integration of primary care management, point of care testing (POCT) and call/recall within the contract
Suffolk County Council has decided not to divide the contract into lots so as to enable flexibility of funding and service provision to address health inequalities across the county throughout the life of the contract.
The estimated scope of the contract is between £16m and £29m. These figures represent the estimated initial contract value through to the estimated total contract value assuming all extensions are granted. These figures are for guidance only and are subject to change.
The initial contract period is 5 years and at the end of the initial contract term at the Councils discretion this contract may be extended by up to an additional 2 years. This extension will be subject to satisfactory performance and availability of funding. This may be followed by a further extension of up to an additional 2 years (at the Councils discretion) at the end of the first extension.
