This service covers the invitations (call-recall) element of the Suffolk NHS Health Check programme, including the identification of eligible patients, issuing of initial invitation letters, follow-up of invitations, recall for future checks and IT support including clinical system searches.
Suffolk County Council is seeking expressions of interest from suitably experienced and competent call/recall and software providers who can deliver a centralised NHS Health Check call/recall programme whilst providing IT support around call/recall (e.g. help with clinical system searches / data extraction).
The overall aim of the NHS Health Check programme is to help prevent heart disease, diabetes, stroke and chronic kidney disease. Everyone who is between the ages of 40-74, who have not already been diagnosed with one of these conditions, will be invited once every five years to have a health check to assess their risk CVD and will be given advice and support to help them to reduce their risk.
There are 80 GP practices in Suffolk, of which currently 12 practices send their own invites and the remaining 66 practices are part of a centralised call/recall programme. The aim in Suffolk is to offer a health check to 46,831 patients each year (20% of the eligible population). It is optional for practices to opt in or opt out of this centralised call/recall programme and the Council is not in a position to guarantee a set level of engagement however we will support the successful provider in encouraging practices to opt in. If the current 66 practices opt in the provider will be expected to offer roughly 33,000 first invites, 25,000 reminders at three months and 14,000 reminders at six months (data from last year’s call/recall programme 2014/15).
