description

NHS North Staffordshire and Stoke-on-Trent CCGs invite suitably qualified and experienced healthcare Providers to bid for the contract to provide a community anticoagulation ‘One Stop Shop’ service.

It is anticipated that interested Providers will have the capacity and capability to deliver an integrated and seamless anticoagulation service. This will provide, on an equitable basis, a safe and effective diagnosis (DVT), initiation, stabilisation, monitoring, dosing and prescribing anticoagulation service in line with best practice timescales. The Provider will be required to deliver a service for complex, non-complex and stable complex patients whilst maintaining care closer to home.

There is an expectation that the service will be delivered by a Prime Provider and sub-contracted to general practices who wish to deliver the monitoring, dosing and prescribing anticoagulation element of the service to patients.

The contract duration will be three years with an option to extend for two years.

Additional information: This Tender will be done via electronic means using the internet.
