WP3436 - Care Home Trusted Assessor

The Council wishes to procure a Care Home Trusted Assessor service from an organisation with the expertise of working at a senior level in social care delivery. The purpose of the CHTA role is to undertake assessments and re-assessments to facilitate safe and timely discharges from hospital into residential/nursing care homes who volunteer to be part of the pilot scheme.

This will help avoid delays in discharge from Walsall Manor Hospital and neighbouring hospitals (within a 15 mile radius of Walsall). NHS Trusts and local authorities that have adopted the trusted assessor model show that it can improve the experience for the patient and reduce delayed transfers of care.

The contract is for a fixed term of 8 months commencing October 2018.
