Arden & Greater East Midlands Commissioning Support Unit (AGEM CSU) on behalf of Leicester City Clinical Commissioning Group (LCCCG) would like to invite providers to attend a ‘Market Engagement Event’ which will inform the market of LCCCG intentions to commission an APMS Care Home Primary Care Service.
The aim of the service will be to offer a multi-disciplinary primary care service to residents of care homes and possibly housebound patients across Leicester City. It is envisaged that this service will give registered patients the benefits of improved quality primary care services, 8-8, 7 days per week.
The new service is intended to be Doctor led and be staffed by clinicians with a special interest in the needs of the elderly.
The aim of the service is to provide better preventative healthcare advice, improved continuity of care which will lead to a reduction in hospital admissions.
There are 107 care homes within Leicester City CCG Boundary with approximately 2600 beds.
Patient choice will still be at the centre of any changes to the services offered. Patients who choose to register with the proposed new service would be agreeing to change from their current GP practice to the new service.
It is intended that a procurement process will commence in June 2015 with a view for the service to be in place by 1 January 2016.
A Market Engagement Event will take place on the afternoon of Tuesday the 12th of May in Leicester.
The Market Engagement Event will provide;
