People are deemed eligible for Fully Funded Continuing Health care if they are assessed and meet the national determined eligibility criteria. This criteria is detailed within the National Framework for Continuing Health Care and Funded Nursing Care (DOH November 2012 Amended)
Patients who meet NHS CHC criteria have a ‘primary health need’ and typically have care needs that are complex, intense and unpredictable and therefore require high quality care delivered by well trained staff who can provide a flexible and reliable service The provider will be required to work with the Commissioner to ensure that the Service achieves the following outcomes:
