Integrated Diabetes Service

NHS Hillingdon Clinical Commissioning Group (CCG) is developing an Integrated Diabetes Service that will increase the number of patients self-managing and treated in primary care and will improve outcomes for patients of all ages with Diabetes. This work is intended to reduce unplanned attendances and admissions and reduce both secondary care costs and activity.

The CCG is seeking to run an information event to inform the development of the service and is seeking input from organisations with experience in the area of delivering diabetic services to:

• Provide feedback on the emerging service specification.
• To highlight issues and opportunities the CCG may have missed in the developing service specification.
• To help the CCG to be as innovative as possible in shaping the ultimate service.

In advance of the session the CCG invites organisations to answer the following questions:

• What experience has your organisation had with delivering services that are similar to the aims that the CCG are seeking to achieve?
• What dashboards, measures or metrics have you used (please provide examples where practical)?
• How have you successfully utilised IT to support patients suffering with diabetes?
• What other information would you like to share with the CCG in advance of the Information Event?
