Younger Adults Supported Accommodation Short Term Service - Fareham.

UPDATE - Subsequent to publishing the original notice the Authority and Provider agreed not to proceed with this service.

This contract will cover Younger Adults Supported Accommodation Services commissioned by Hampshire County Council.

Younger Adults Supported Accommodation means - Accommodation that has some element of care and support attached to it - this could be from a few hours per week of shared support to 24/7 support with a team of support staff

This service requires a Support Provider who is confident to support individuals with complex needs who may present with behaviours which Challenge.

A management oversight of the Service which understands PBS and can guide and support the staff team and work with the involved Health Professionals to deliver safe and effective support

Ability to implement safe and effective systems to monitor and respond to the health needs of the individuals and ensure that support is being delivered consistently, correctly and recording is being completed effectively.

The provider is expected to meet a range of care and support needs from prompting with personal care and daily living activities to full assistance of 1:1 or 2:1 support workers for all personal care and daily living activities, including hoisting and the use of other equipment to support with mobility needs
