Sheffield City Council will shortly be implementing a community-based prevention model that is estimated to reduce the pressure on health and care services by a minimum of £10m (Net Present Value) over 10 years. We have secured Transformation Challenge Award funding from DCLG to make an enabling investment in a large-scale rollout of a smaller pilot - helping us bring forward a major transformation and deliver improved outcomes in Sheffield. To assess the effectiveness of the approach Sheffield has developed we are seeking an evaluation partner to complete an evaluation and contribute to the development of the city's evidence base to inform future delivery of prevention work
