NHS Heywood, Middleton and Rochdale Clinical Commissioning Group (NHS HMR CCG) are looking for suitably qualified Providers to submit tenders for a Bio-Psycho-Social Pain Management Service.
The service is for adults Aged 18+ and registered with GP in Heywood, Middleton and Rochdale and will provide flexible opening and availability, delivered Monday to Friday but also offering evening and weekend appointments (which should account for a minimum of 20% of available appointments).
The provider will be expected to locate the service where possible within the NHS Prop Co facilities outlined in this service specification.
The successful delivery of this service model is dependent on the three tiers working in a collaborative shared-care person-centred way.
Tier 1: Primary Care (Mild Impact) (Not in this scope)
Tier 2: Community Pain Management service (allied health professional led) (In Scope)
Tier 3: Specialist pain management service (medically led) (In Scope)
The service aims and objectives are:
- Single point of access to pain management via the NHS HMR CCG Referral Gateway
- Multi-disciplinary team the patient is seen by the right person, in the right place and at the right time in the appropriate tier of care
- To provide early, effective interventions to minimise disability
- To provide ongoing education and support to primary care and other non-pain specialist providers in the management of persistent pain
- To support patients in understanding their quality of life in the context of their persistent pain
- To support and signpost patients to gain a good level of information and health literacy about persistent pain
- To encourage and support patients to become actively involved in their self-management as detailed in their jointly developed Care Plan
- To safely optimise the choice and use of medicines in accordance with National and local medicines management protocols and formulary, and offer medication reduction programmes e.g. opiate dependency
- To increase independence and quality of life for patients, and their families/carers where realistic or possible
- To review and redesign the service in response to feedback from patients, key partners and evidence based practice
- Implement opportunities for ongoing pain management training and development of healthcare professionals
- To provide a single portal of pain self-management resources to clinicians and patients
- To provide a shared care network of local service provision to provide care closer to home
- To act as an integrated provider of persistent pain management services to maximise the patient’s pathway relating to health & non-health services e.g. voluntary sector/mental health provision/holistic therapy provision.
The contract duration will be for 5 years with a Service Commencement date of 1st August 2016.
The tender process will be conducted as a single stage open procedure similar to that of an Open Process.
