Telemedicine Service for Care Homes on behalf of NHS Croydon CCG

NHS Croydon Clinical Commissioning Group has awarded a contract to Immedicare LLP (who provide services in partnership with Airedale NHS Foundation Trust).

This is for provision of a fully managed virtual support telemedicine service that gives care homes 24/7 virtual access to a range of clinicians at levels of specialism and seniority appropriate to interventions needed to keep residents out of hospital, away from primary care and improve the health and care outcomes for care home residents. This contract will be for the provision of the service in up to 80 homes - both nursing and residential.

This service also connects seamlessly to primary care, community services such as rapid response and district nursing, ambulance services, and others, in order to provide clinical triage recommendations for care, and onward referral where needed, all with the security of clinical capacity provided by an acute hospital. The service also makes available 24/7 technical support for service users (the care homes).

Additional information: The provider was selected for direct award from Technology Enabled Care Services SBS framework, specifically in Lot 4 - Scheduled Remote and On Demand Services, on the basis of being the only technically capable provider able to meet in full the CCG's specified requirements.
