description
The Commissioner (Coventry City Council and Coventry & Rugby Clinical Commissioning Group) is looking to contract with providers for the delivery of long term home support. This will be for Adults and Children that meet national eligibility criteria for social care and Adults eligible for non-complex health funded Continuing Health Care at home.
Support will be for eligible adults and children/young people and will provide personalised care and support to people in their own homes to enable individuals to live independently. The service will also assist carers/parents to maintain their care role.
The service will be let in 9 lots. There will be 7 areas for general delivery aligned to Coventry and Rugby Clinical Commissioning Group (CCG) GP clusters and 2 City Wide clusters covering mental health and learning disabilities.
