description
The Council will commission 3 Supported Living services under one contract for adults with learning disabilities. The contract will be for 2 years with an option to extend for 1 year.
Two services will be transitional; working with the Service Users to enable greater independent living skills and increasing their suitability for move on to Independent Living Accommodation. The expectation is that the Service Users will be ready to move on to their new accommodation by the contract end.
The remaining service will be required to work with service users preparing them for moving on to more fit for purpose supported living accommodation in the future. The expectation is that this goal will be achieved by the contract end.
During the contract term the supplier will be required to:
•Review each Service User's allocation of support hours and identify ways of achieving the best outcomes and further saving/efficiencies.
•Work with Service Users to provide support in sourcing more Independent Living Accommodation as their level of independent living skills increases.
•Work with the Council and housing suppliers to facilitate the move on process and plan the transition pathway.
The contract start date is 1.12.2018.
Transfer of Undertakings (Protection of Employment) Regulations 2006 (TUPE) may apply to this contract.
