The Council is looking for providers to deliver the Making Connections Walsall Programme. The MCW programme is about tackling loneliness and social isolation, improve wellbeing and reduce preventable health and social care service usage. It will take a Social Prescribing approach to improve the social connectedness of older people across Walsall.
The Making Connections Walsall Programme is targeted at meeting the social needs of the older adults who are lonely and socially isolated NOT SOCIAL CARE or MEDICAL needs.
Providers with a local understanding of Walsall's Older population and experience of successfully supporting vulnerable older people to address their social needs and access community initiatives.
