West Dunbartonshire Health and Social Care Partnership is developing a service which meets the needs of older adults aged 65 and over who are going through diagnosis, those who have received a diagnosis and those who have been living with a diagnosis of Dementia for some time.
The Brain Health and Dementia Resource Centre contract will provide post diagnostic support through the provision of Post Diagnostic Support Link Workers who will support individuals for a minimum of 12 months post diagnosis, this is in line with the dementia post diagnostic support local delivery plan standard. The service will support individuals to have the necessary assistance and support to achieve and sustain their individual outcomes.
