SERVICE SUMMARY
NHS South West London Integrated Care Board (SWL ICB) is undertaking pre-market engagement to inform the potential commissioning of a NHS High-Risk Communities Targeted Lifestyle Behaviour Service for North Croydon. The proposed service will deliver targeted, evidence-based interventions focused on weight management, smoking cessation and alcohol abstinence for residents at highest risk of preventable long-term conditions and premature mortality.
The service will focus specifically on high-risk populations living in North Croydon. The intention is to complement the existing universal prevention offer commissioned by Croydon Council by providing a targeted NHS-funded programme aimed at populations with the greatest unmet need and potential for measurable health impact.
The service will deliver structured behavioural interventions, targeted outreach, and coordinated referral pathways with primary care, voluntary sector partners, and existing public health services to ensure residents can access the most appropriate support and that duplication of services is avoided. The programme will contribute to improving cardiometabolic health outcomes, reducing smoking prevalence and alcohol-related harm, and addressing health inequalities in line with NHS Long Term Plan prevention priorities and the Core20PLUS5 approach.
The total indicative financial envelope for this service is £184,539 per annum.

System Alignment and Strategic Context
Croydon operates within a collaborative health and care system, with both NHS and Local Authority partners contributing to the prevention and health improvement agenda.
The London Borough of Croydon Public Health team is progressing plans to procure a borough-wide, universal Adult Healthy Lifestyle Service. This service is intended to provide broad access to weight management, smoking cessation and alcohol reduction for Croydon adult residents.
In parallel, the NHS-commissioned High-Risk Communities Targeted Lifestyle Behaviour Service for North Croydon is being designed as a targeted intervention, focused on residents at higher risk of poor health outcomes, in line with Core20PLUS5 principles and local population health intelligence.
These two offers are intended to be complementary and mutually reinforcing, rather than duplicative. The expectation is that providers will:

| • | Establish clear referral, signposting, and escalation pathways between services |

Commissioners recognise that the multi-component nature of this service may be well suited to partnership or consortium delivery; therefore, providers are invited to indicate whether they consent to their details being shared with other registered organisations to facilitate discussions on potential partnership opportunities.
