Due to the approaching expiry of an existing contract Northumberland County Council are advertising to seek tenders from suitably experienced and qualified organisations with the intention of creating an improved framework of multiple Suppliers (on a non exclusive basis) who are able to deliver support for disabled children and young people within their homes and community. This will be in line with assessed levels of need and may form part of a continuum of care.
It is intended that the service shall:
| ● | Build capacity within the home environment and support families to develop strategies and skills to assist with the management of the child’s needs and/or condition(s); |
| ● | Enable children and young people to access social and leisure opportunities, assisting them to integrate into local universal and specialist community resources; |
| ● | Enable family carers to take a break from their caring role. The family carers may choose to remain in the home or to take a break outside of the home. |
Delivery may include the provision of day, evening and weekend activities for the child or young person and can take place in the child's own home, the home of an approved carer or in a community setting.
