The Services shall be made available to People with a Disability who are 16 or over and who are resident in York who are assessed as meeting the Care Act Eligibility Criteria and Children and Families Act 2014. This does not preclude other Individuals using the Services but funding for provision of Services shall be agreed with the person, their carer or other agencies / individuals prior to the provision of the Services.
Detailed elements within this specification are for:

| ii. | Support at a base the Person has chosen to attend where different opportunities for Community activities are available |

| v. | Volunteering, employment and peer support and Person led activities |
| vi. | Personal and / or practical care to those that struggle to access their community independently |
