This Invitation To Tender (ITT) is for procurement (the Procurement) of a Medicines Optimisation for Care Homes Service (MOCHS or the Service) by NHS Hastings and Rother CCG (as Lead Commissioner) and NHS Eastbourne Hailsham Seaford CCG (as Associate Commissioner) (together referred to as the Commissioner).
Service
The Service is outlined below and more fully described in the draft specification included in the tender documents.
The Service is aimed at optimising the use of medicines for older people living in care homes. The Service aims provides medicines optimisation (MO) and deprescribing in Care Homes to improve patient safety and quality of life by:
- Creating additional capacity in Primary care to address the complex needs of our frail elderly patients
- Tackling the adverse effects of inappropriate polypharmacy
- Avoiding unplanned admissions to hospital
The Service will contribute to the multi-disciplinary care of frail elderly patients by providing a comprehensive clinical pharmacy review of medicines as part of the care planning process. The service will also provide technical support to care homes on the safe and secure handling of medicines within the homes.

Bidders wishing to participate in the Procurement must declare an Expression of Interest (EOI) and submit a bid as detailed in MOI.
The EOI and bid response must arrive before the deadline detailed in the MOI. Failure to return a completed bid by the deadline above will normally result in the disqualification of the Bidder from participating in the Procurement.
