NSC has a statutory responsibility to provide domiciliary care services to clients who meet the Care Act 2014 eligibility criteria. This is currently delivered under via Support to Live at Home contracts which were recommissioned over the last 3 years. The Council is now in a position to be able to identify areas where these contracts need support to deliver effective and responsive services.

The Council works closely with the CCG to support services either commissioned directly through the Council or through their contracted Provider the North Somerset Community Partnership. This includes support to the Rapid Response Clinical Hub who coordinate North Somerset's responsive community nursing service and work to prevent unnecessary hospital admissions.

This is a Framework Contract is for the provision of domiciliary care on a short term basis via a Block arrangement for the adult population will comprise of a service that supports 3 Service Areas;

  • Hospital Discharges
  • Hospital Admission Prevention
  • End of Life Care
