NHS South East Staffordshire and Southern Peninsula CCG are looking to commission an 18 month pilot for a Primary Eye Assessment & Treatment Service (PEATS).
A draft service specification will be sent to all parties who express an interest and this will outline the commissioner’s intentions for the service.
This is a pilot and the commissioner intends to work closely with the provider to shape the service and ensure continuous improvements are made in line with patient and GP feedback. The commissioners reserve the right to stop the pilot if the outcomes are not being met after the first 6 months.
The Provider shall adopt an integrated service model with the overall aim of providing a timely assessment of needs of a patient presenting with an eye condition who would otherwise present in a secondary service, either as routine outpatient or as an attendance at an emergency eye hospital.
The overall aims and objectives of this service are:
| • | To act as a single point of access for Service Users presenting with an eye condition. |
| • | To include the specialist triage of ophthalmology referrals to ensure patients are seen in the right place by the right person at the right time and actively manages inappropriate referrals through education and support. |
| • | To ensure the safe, appropriate and timely assessment and treatment of all minor eye condition’s (other than procedures requiring General Anaesthesia) delivered in a community setting, within 4 weeks of referral. |
| • | To ensure every referral follows an agreed pathway that optimises both the patient outcome and use of scarce resources. |
| • | To implement choice at the point of (onward) referral |
Additional information: Service Outcomes:
| • | Service Users presenting at the service shall be managed and discharged by the service. |
| • | To demonstrate that patients referred on to secondary care receive a diagnosis or intervention |
| • | To improve access to ophthalmology services and equity of provision by ensuring referrals are triaged within 48 hours of referral and patients have received a face to face appointment within 4 weeks for 96% Service Users. Urgent patients to be seen on the same day of referral. |
| • | To ensure all patient experiences are reported as positive and that they would recommend the service to family and/or friends. |
| • | To reduce the number of referrals into secondary care services by moving care closer to home, into the community, baseline to be used shall be 2013/14 activity rates. |
| • | To utilise the skills of primary care optometrists, GPs with special interest in ophthalmology, ophthalmic medical practitioners, and specialist nurses. |
| • | To streamline patient pathways and encourage integration with secondary care providers |
| • | To improve education for GPs around eye conditions |
