<b>Contract Title:</b> Fit 4 Life: Specialist Adult Weight Management <b>Contract Number </b> ESCW (PH) 4557 The London Borough of Tower Hamlets (LBTH) is seeking expressions of interest from interested organisations for the above services and / or goods. <b>Description:</b><b></b> The London Borough of Tower Hamlets is looking for an innovative and experienced organisation to lead the delivery of a Specialist Adult Weight Management Service as part of the Fit 4 Life Programme <b>The Fit 4 Life Programme</b><b></b> The Fit 4 Life Programme will support local adults (18+) to make sustainable improvements to their diet, physical activity levels and weight, targeting: · Adults with or at high risk of type 2 diabetes and cardiovascular disease · Adults who are obese with co-morbidities or severely obese · Adults with other long term conditions who can benefit from Fit 4 Life services. The programme will have 4 separate but linked services: · The Fit 4 Life Centre - will receive referrals, provide assessments, personalised goal setting and action plans, help clients to access other services and activities based on the assessment and action plans, provide on-going motivational coaching for a year and follow up at 3, 6 and 12 months · Fit 4 Life groups - for adults who have identified group support as part of their action plan · Specialist weight management - for adults with BMI >35 with high risk co-morbidities or BMI > 40 · Specialist disabilities service - for adults who need intensive 1:1 support The approach across the whole programme is to support local people to embed behaviour change into their everyday lives with an emphasis on sustainability, self-direction, peer support and making use of existing community activities. .<b></b> <b>Specialist Adult Weight Management</b><b></b> The specialist adult weight management service will: Provide multi-disciplinary specialist assessments linking with the Fit 4 Life Centre Deliver 12 week multi component weight management programmes through 1-1 and group support Enable participants to achieve independent and sustainable behaviour changes, e.g. through peer support, home and community activities Help participants to achieve 5-10% weight loss at 3 months Refer participants to other parts of the obesity care pathway, working closely with the metabolic clinic, the Fit 4 Life Centre and the Fit 4 Life Groups Support the development of guidance, education and training on obesity and weight management in Tower Hamlets working closely with the Fit 4 Life Centre.
