NHS Barnsley Clinical Commissioning Group is looking for a supplier to deliver a new model for Social Prescribing services. The new model will enhance the health and wellbeing of the residents of Barnsley and help decrease health inequalities by providing a liaison service to support patients and social care clients to access non-medical sources of support in the community.
