The NHS Health Check programme is a mandatory public health service for the local authority. The NHS Health Check is a systematic vascular risk assessment and management programme to help prevent various cardiovascular diseases (CVD) including heart disease, stroke, diabetes, dementia and chronic kidney disease (CKD). The eligible cohort includes people between 40 to 74 years of age who do not have any diagnosed CVD at the time of the check. The Check is offered once every five years to assess their risk and offer support and advice to help them reduce or manage that risk. Smoking remains the single largest preventable cause of premature death, disease, disability and health inequalities in the UK.

The aim of the MED is for LBH to explore a range of options around delivery models to deliver both targeted and opportunistic NHS Health Checks including a systematic call and recall element and A Smoking Cessation Services for the residents of LBH

Additional information:

Specific objectives of the NHS Health Check service include:

  • To offer an NHS Health Check to 20% of the eligible population every year as part of a 5 year rolling programme with an uptake level of around 75% ( a target trajectory will be set)
  • To enable the early detection of hypertension;
  • To enable the prevention and early detection of diabetes;
  • To enable the early detection of CKD;
  • To identify individuals with a high risk of future cardiovascular disease;
  • To increase awareness of the link between cardiovascular disease and vascular dementia;
  • To initiate the appropriate medical management of newly diagnosed chronic diseases;
  • To work collaboratively with individuals with a high risk of a future cardiovascular event and offer them on-going support through referral to one or more of the following local lifestyle intervention services, Stop smoking service, Alcohol service, Exercise on referral programme, Weight management service.

Hillingdon Stop Smoking Service strives to support at least 5% of the estimated smoking population. It provides a quality, evidence based service to Hillingdon's residents, facilitated by strong partnership working with GP practices, pharmacies and other healthcare settings. The programme has consistently achieved a good conversion to quit rate by targeting priority groups such as smokers from routine and manual occupations, pregnant smokers, young families, teenagers and adults with long term health conditions.

The Hillingdon Stop Smoking Service adheres to guidance from Public Health England, Department of Health and the National Institute of Health and Care Excellence (NICE).
