NHS Great Yarmouth and Waveney CCG wishes to enter into a number of framework agreements for the provision of specialist home care for patients qualifying for Continuing Healthcare (CHC).
Contract commencement date - The contract will commence on the day we send the formal award letter accepting the chosen tender - Contract award date estimated to be 19th January 2016.
Required Service Commencement Date - 14th April 2016. Term - 48 months from the Required Service Commencement Date.
