NHS West Yorkshire integrated Care Board through Calderdale Cares Partnership as the Contracting Authority is seeking to identify a suitable partner to provide primary medical services under contract to a practice population of 2,850 patients residing in or around the Todmorden locality. The population is currently served under an APMS contract which will expire at the end of March 2024.
Under the contract the successful Bidder(s) will be required to provide high quality primary medical services to the practice population referred to above and ensure continuity of service provision to patients.

The Calderdale Cares Partnership requires that the following is understood and confirmed by potential bidders as the basis of their interest:

Additional reimbursements relating to Quality Outcome Framework (QOF); Direct Enhanced Services (DES); Local Enhanced Services and Premises Directions etc., will apply as per prevailing national guidance and achievement criteria. |
| - | Should an APMS contract be awarded it is expected that the initial contract period will be 5 years with further extension options. |
| - | List size of approximately 2,850 patients |
| - | The Calderdale Cares Partnership would expect services to be provided from the current premises located within Todmorden Health Centre, Lower George St, Todmorden OL14 5RN |
| - | Core/essential and other services to be provided at the Todmorden Health Centre location from 8:00am to 6:30pm (Monday to Friday) |
| - | The Calderdale Cares Partnership would expect practice membership and active participation in the appropriate Primary Care Network |
| - | The Calderdale Cares Partnership would require any potential bidder to demonstrate the capacity and capability to provide the service from 1st April 2024. |
Based on the above, Calderdale Cares Partnership is keen to receive expressions of interest from suitably qualified potential bidders.
