The East Riding Safeguarding Adult Board is undertaking a Safeguarding Adult Review (SAR) in line with the Care Act 2014. Specifically this relates to the issues surrounding the death of an adult who had multiple complex physical & medical health and social care needs and complex learning disabilities.
As part of this review the Board requires an experienced independent report overview author.
