NEW Devon Clinical Commissioning Group are seeking proposals to provide education services for newly diagnosed type 2 diabetes patients in their Eastern locality. The service should provide a good understanding of what the patients diabetes means for them, how to cope with their diabetes and increase their knowledge and understanding to reduce future diabetic related complications.

The aim of patient education is for patients with diabetes to improve their knowledge, skills and confidence, enabling them to take increasing control of their own condition and integrate effective self-management into their daily lives. Diet and exercise are the foundation of any diabetes management plan. High-quality structured education could have a profound effect on biomedical outcomes, and can significantly improve quality of life and satisfaction.

The contract period is planned to be 3 years with an optional extension of 2 years.
