The purpose of this service is to offer a programme of assessment and support to meet the specific needs of young carers aged 5 to 18 and their families in Merton.
All children referred will be assessed in accordance with recent legislation and statutory guidance. Assessments will determine level of need and support offered with priority given to those at enhanced or specialist levels on the child well being model using a partnership model.
