The Worcestershire CCGs are currently reviewing their CHC Fast Track Funded End of Life Care in Nursing Homes and have made a decision to commission a Pilot for care with Designated Nursing Homes within each of the three CCG areas.
The CCGs initial focus is to invite tenders from relevant providers in the market who have current experience of delivering End of Life Care in the community within a Nursing Home environment.
Interested and capable providers must be able to provide services for patients registered with Worcestershire GP Practices within the relevant CCG area.
The CCGs will run a transparent process, ensuring all bidders (where applicable) are given equal opportunity.
The CHC Fast Track End of Life Designated Nursing Home service will deliver
| - | High quality end of life care for patients that choose to end their life in a Nursing Home setting |
| - | Provide rapidly accessible care thus preventing delays with discharge from hospital. |
| - | Provide support to family members |
| - | Prevent avoidable admissions and readmissions to hospital |
All staff will be trained via an evidence based End of Life Care programme.
The service will provide one primary function:
To deliver a personalized high quality service in the delivery of end of life care, responding to the needs of patients and family members.
The contract term will be for an initial pilot of 12 months, with an option to extend for a further 12 months.
The CCGs are looking to award a number of contracts to cover the 3 CCGs.
The indicative number of new packages by locality per year are: SW=196, R&B=72 and WF=28.
The aim is to award contracts per CCG lot as follows:
| - | NHS Wyre Forest CCG - up to 2 contracts |
| - | NHS Redditch & Bromsgrove - up to 4 contracts |
| - | NHS South Worcestershire - up to 6 contracts |
The CCGs reserve the right to award in part or in whole. The CCGs reserve the right not to proceed with award.
