description

This service must take an holistic approach to reducing both social and digital isolation for older people and their carers in Dacorum. The service should primarily (but not solely) focus on providing support for those without social care needs. Through the provision of a variety of activities this service will seek to improve resilience and independent living and the main areas of focus should include but are not limited to:

Accessing digital equipment
Using technology to communicate
Social Activities
Health and Wellbeing initiatives
Community Engagement
Intergenerational engagement / activities
Life Skills
Signposting and referral pathways
Supporting DBC tenants to have warm, weather tight, safe and secure DBC properties (DBC tenants only)

This service is required to provide support residents across the Borough and will need to work in partnership with Elderly Person Dwellings. The Service Provider will need to ensure all relevant residents across the Borough (in both central and rural areas) are able to access the service and any related activities if the outcomes are to be achieved.

To ensure this service is holistic, service providers will be required to work in partnership with relevant organisations which could include but are not limited to: Hertfordshire County Council, Public Health, Herts Valley Clinical Commissioning Group, other voluntary organisations, Dacorum Borough Council.

Further details of the Service Requirements are provided in the tender documents which are available from the e-tendering portal www.supplyhertfordshire.uk

Please submit the completed Tender document via the Council’s e-tendering portal no later than 12 Noon 11th May 2016. Any tender submitted either manually or by email will not be accepted.
