North Yorkshire County Council and Scarborough and Ryedale Clinical Commissioning Group have been working together on plans to improve the health of our population. Our joint plan, Ambition for Health, sets out our priorities for healthy lives, care at home and sustainable services. We want to develop Health and Social Care provider services together, so that people can remain as independent and healthy at home for as long as possible.
The health and social care needs of the population of Scarborough and Ryedale are changing. A combination of an ageing population with increasing co-morbidities, the changing expectations of our population, the growing advantages and expectations of technology, and a predicted increase in demand will all place additional pressures on this health and social care economy
As part of this work we want to engage and consult with stakeholders including staff in the community services. On Monday 21 November we will be holding two workshops facilitated by Claire Old from the ECIP team. The purpose of these workshops will be to share our thinking to date and test out with staff and stakeholders the model of integrated care that we are developing based on the feedback.
