Dorset County Council wishes to purchase an Information, Advice, Signposting and Support Service for Adults who have had a Stroke in Dorset. This service currently exists and offers targeted, personalised, support to people who have had one or more strokes to support their wellbeing and promote continuing independence. The Care Act 2014 continues the movement of social care to a preventative, personalised and proactive care approach and this service meets that aim through proactive, preventative and enabling support from a skilled advisor. There are existing NHS acute and community services delivering a range of health interventions for people who have had a stroke. Dorset County Council provides social care models of care and support for people who are eligible for services. Existing community and voluntary organisations also provide support to stroke survivors that range from expert advice to general community activity. This service will not duplicate those services but is intended to compliment the work of public and community organisations and make sure that timely expert advice to help individuals manage their daily lives and connect with other organisations and groups is available at the point in an individual's recovery when they want it. This service is for adults of 18 years and over, resident within Dorset (excluding Bournemouth and Poole). Further information can be found in the Service Specification (Appendix 1).
