North Tyneside CCG are creating an opportunity for potential providers to engage in respect of a new model of Chronic Obstructive Pulmonary Disease (COPD) and Cardiovascular Disease (CVD) rehabilitation to deliver person centred outcomes that enable people to live better quality and independent lives.
North Tyneside CCG wish to commission a service that enables more people with COPD and/or CVD to: -
Live more active, independent lifestyles after the Get Well Stay Well programme;
Feel more confident, in control and able to cope with their lives;
Be more socially connected with their communities;
Be role models and supporting a wider community /families; and
Use secondary health care services less.
The New Model
The local model has been developed in response to patient, public and primary-care feedback, as well as utilising local intelligence on levels of need, current referral patterns and potential opportunities based on national and regional evidence.
The delivery model will aim to provide a service which: -
Provides a personalised and menu based service (that may be applied to other long term conditions in due course);
Addresses the spectrum of need from early disease to later stage /complex needs;
Is consistent with the emerging, local extensivist model of care in North Tyneside (increasing the potential impact of primary care and the menu of options the eventual provider can offer patients to manage their condition more effectively);
