Community Assessment Beds (Health and Social Care Village Beds)

Kent Community NHS Health Foundation Trust (KCHFT) is seeking tenders from suitable experienced providers who can deliver a Health and Social Care Village Service.

The Health and Social Care Village Service supports the discharge of patients who have been assessed in an Acute hospital setting as safe to be discharged from an Acute Hospital environment and into a Nursing Home. Here, they will receive a further assessment of need, rehabilitation and a period of recovery while developing a plan of care and onward referral if required.

This service is part of the East Kent KCHFT portfolio of Services which seek to improve Delayed Transfer of Care and reduce Hospital admissions / readmissions.

KCHFT is looking to work with social care providers, who are registered with the Care Quality Commission (CQC) for the provision of residential nursing care, to support the identified care needs as part of the discharge plan of care for the patient.

The initial delivery model, per block / per area, consists of a volume of required beds. Providers should refer to the Tender pack for full details of the Trust's requirement.

The Contract will run until the end of March 2021, unless terminated in accordance with the Terms and Conditions. The Trust may extend the Contract Term by 12 months (to March 2022) at its sole discretion.
