Derbyshire County Council is inviting tenders to provide a Carers Assessment and Support Service for Derbyshire (excluding Derby City). The Council is offering a single contract. The successful provider might sub-contract some or all of the service, or be the lead provider for a Consortium, or deliver the service in its entirety.
The Council are seeking a provider with a proven track record of managing and delivering high quality carer assessment and support services.
The successful provider will be required to evidence outcomes that result in changes for individuals receiving the service, and outcomes.
