High Intensity Team for Care Homes - Milton Keynes CCG & Milton Keynes Council

NHS Arden and Greater East Midlands Commissioning Support Unit is seeking expressions of interest for the Provision of a seven day multi-discipline team to support Care Homes’ staff on behalf of Milton Keynes CCG & Milton Keynes Council to avoid Emergency Admissions and A&E attendances of patients/residents. The team is to be Nurse led with access to other disciplines as required (including but not limited to Physio/Occupational Therapy, Mental Health nursing, specialist community services and Pharmacy as deemed appropriate) to cover the Care Homes in Milton Keynes. The overall objectives of this service are to improve resident experience, support the development and capability of care home staff and reduce secondary care costs.

Milton Keynes CCG and Milton Keynes Council would like to invite interested parties to attend a bidder information day for the procurement of High Intensity Team for Care Homes for Milton Keynes, which is part of the CCG’s and the Council’s Commissioning Intentions. The workshop will set out the potential opportunities for parties who may be interested in providing these services for Milton Keynes.
