The overarching purpose/output of this project is to develop a strategy for personalisation for Tower Hamlets for the next 3-5 years. (Pro Contract Link: https://procontract.due-north.com/Login/) This strategy will need to include and/or be informed by:
| 1. | A mapping/consolidation exercise of work that has happened to date |
| 2. | A quantitative and qualitative evaluation of the work that has happened to date |
| 3. | A vision for personalisation for Tower Hamlets, which has been developed and agreed with all key partners in the system |
| 4. | Priority areas of focus for future years, working towards the vision that has been agreed |
| 5. | Details on the level of resource that may be required to deliver the strategy going forward, for example, in terms of programme management costs |
| 6. | Details on how PHBs can be funded on a sustainable basis going forward, for example, through extracting money from existing contracts |
| 7. | A delivery plan, to accompany the strategic objectives |
| 8. | Measures to track progress of the strategy, once implementation has commenced |
| 9. | Some outline modelling of the potential for the programme to improve health and well-being outcomes and/or reduce pressure on other parts of the system e.g. through a reduction in primary and secondary care utilisation |
