The CCGs have been considering arrangements for the future commissioning and delivery of affordable and sustainable healthcare fit to meet the future needs of their communities in line with the Five Year Forward View.

Informed by feedback from patients, the CCGs have concluded that moving towards an accountable care system model is the right model for Bradford. The CCGs intend to work with local healthcare providers and the Council to determine the scope, outcomes and structure of the model through a structured collaboration process. The model will deliver community services, integrated with primary care and local acute services.

The CCGs anticipate a contract with a single entity but this will be explored and tested as part of the structured collaboration process. The intent is for all health provider members to have responsibility for delivery, system management and sharing income risk through governance arrangements; this will be reviewed in the structured collaboration process.

The CCGs will refine the scope, specification and governance / delivery structure of the accountable care system as part of the structured collaboration process, which will start on 22 September 2016. This will build on work already undertaken with diabetes services, so that the model delivers community intermediate care services that will be fully integrated with primary and acute care - services are currently anticipated to include are included on the CCGs website at http://www.bradforddistrictsccg.nhs.uk/news/out-of-hospital-care---commissioning-event/ (the "Services"). The Services maybe subject to change as the CCGs' development of an ACS model develops and patients will play a key role in the development of the ACS.
