WCC - Warwickshire Carers Wellbeing Service - AWARD

Warwickshire CC are looking for a single provider to provider a Carers Wellbeing Service (to adult carers aged 18 and above, caring for another adult). The service will focus on empowering carers to enable them to balance their caring role with a life outside of caring, whilst maintaining and promoting their health and wellbeing.

The Service will support the Council's duties towards carers under the Care Act 2014 which includes: the provision of information and advice; the promotion of well-being through the delivery of preventative services; the identification and assessment of carers and the provision of support to those carers eligible for support. The Service will be accessible to people funded by WCC and self-funded

The Provider will have the ability to manage referrals, caseloads, assisting carers in assessing their support needs and develop exit strategies from the service, signposting them to other services that are more appropriate for their circumstances / to meet their needs.

In circumstances where the carers assessed, eligible needs are met through the provision of goods or services that are not part of this specification, the provider will be expected to assist the carers to manage and support the carers support package if required.
