The service aims to provide highly specialist learning disability health services to improve the physical and mental health and welling being for adults with learning disabilities living in Wandsworth. The service consists of a multidisciplinary team (MDT) of professionals who work together to provide specialist input to adults with Learning Disabilities. Each professional provides a range of assessments and interventions, some of which will be unique to the professional group and others which will be shared with other professions. The service provides community nursing, dietetics, dysphagia, exercise management, Occupational Therapy, physiotherapy, psychological therapy, and Speech and Language Therapy

Additional information: This contract award notice provides notification that NHS Merton (or Wandsworth) Clinical Commissioning Group has agreed to directly award an interim transitional contract for the provision of Community Learning Disability (LD) Services to South West London and St George's Mental Health NHS Trust, for a period of 24months (until March 2021). This interim contract is considered essential to ensure a continuity of essential service provision to the local service users whilst a new, broader integrated mental health provision, inclusive of LD is considered and specified. The CCG has assessed that the new provider identified is the only capable provider at this time with sufficient capability and capacity to put into place the urgent service provision required whilst the commissioning review is undertaken. This contract will provide an interim transitional and stabilising arrangement with the provider being assessed to be a safe and logical destination to transfer the services. The contract duration will provide the CCG sufficient time to develop its future commissioning intentions and an integrated health model including Community Learning Disability Health Team and the Mental Health Learning Disability Team, ahead of engaging with the market during 2019/20 to inform the most appropriate approach to establishing a future, substantive solution from April 2021 onwards
