Children, young people and adults with a learning disability and/or autism have the right to the same opportunities as anyone else to live a satisfying and valued lives, and to be treated with dignity and respect. They should have a home within their community, be able to develop and maintain relationships, and get the support they need to live healthy, safe and rewarding lives.
Thinking about the future is an important aspect of person-centred care planning for all people within inpatient settings, which should aim to build on the person's strengths and skills and help them consider what they want most in life as they get older. This would complement C(E)TRs by providing greater clarity about the person's aspirations, assisting commissioners to ensure suitable services are developed.
Life planning is a key element to support delivery of the programme and should include:
| • | Building friendships and support networks: describing the benefits of having more people involved in the person's life and ideas for achieving this. |
| • | Keeping an eye on things: how to ensure the person has a good life and is well supported. |
| • | Talking about difficult subjects: ideas about how to talk about subjects that can be difficult to raise. |
| • | Making financial plans |
| • | Housing and support: information about the different housing options for the person and how to get the support they need |
| • | Making a plan for emergencies |
| • | Making a plan for the future: drawing together the person's ideas to make a plan for their future |
The outcome of this work will be to have a clear person-centre plan for each of the identified people, with a clear understanding by the person, their family/carer/advocate, commissioner and provider of the next steps in order to achieve their pathway to a home in the community.
