Brief description:

We are looking for an experienced Social Worker registered with Social Work England who has management experience and is keen to develop new ways of working and wants to make a difference.

Our vacancy is for a Team Manager within our busy Hospital 2 Home Team which is an exciting opportunity for an individual who is flexible, innovative and has a proven track record within a similar setting.

Duties of this Social Work Team Manager Role:

• oSupport the Operations Manager within the 7- day Hospital 2 Home Team.
• oBe responsible for the leadership and management of the Social Work Team, ensuring that the Discharge to Assess Processes are embedded in to practice.
• oWork from various locations and oversee the timely discharge of people from hospital or intermediate care settings.
• oAlongside the Operations Manager, be responsible for 6 Social Care Lead Officers and provide case direction and supervision to these staff.
• oWork in partnership with the NHS and the ICB.
• oHave a strong customer focus.

Requirements:

• oAdults Enhanced DBS
• oSocial Work England Registration
• oSocial Work Degree, DipSW, CQSW or equivalent.
• oFull drivers licence and/or the ability to travel around the borough. It may be a requirement of the post that you use a vehicle for undertaking visits, including transportation of clients or small items of equipment.
