Tier 3 / Tier 4 Weight Management Services Hampshire CCG

Fareham & Gosport CCG, North East Hampshire and Farnham CCG, North Hampshire CCG, South Eastern Hampshire CCG, West Hampshire CCG and Southampton City CCG are considering collaboratively commissioning Tier 3 and Tier 4 Specialist Weight Management Services.

Tier 3 Weight Management Service, consisting of a Multi-Disciplinary Team providing an intensive level of input to patients unsuccessful in achieving or maintaining weight loss at Tier 2 with BMI >35 with comorbidities or BMI >40 without. The annual contract value for the services is to be confirmed. The anticipated annual contract value is likely not to exceed £350,000 (based on current funding for the service).

Tier 4 - Specialised Complex Obesity Services (including bariatric surgery) for patients with BMI >45 or consider >30 if diagnosed with Type 2 diabetes in the last 10 years. The annual contract value is to be confirmed.
