NHS Bradford City CCG and NHS Bradford Districts CCG Skin Surgery Service AQP Contracts

The provider will offer a community-based skin surgery service will integrate fully with primary care, the patients’ registered GP and the local hospital based MDTs where appropriate.

The provider can elect to provide all or any one of the following specifications of service, namely:

The commissioners requires a responsive community-based service model with the overall aim to provide routine, clinically appropriate diagnosis and treatment of acute conditions requiring a certain level of skin surgery and support for primary care clinicians in the management of patients.

The service will have the following aims, providing services for patients with conditions requiring skin surgery that:

• c)Aid early diagnosis and management of patients with a condition requiring skin surgery and avoid unnecessary referral to secondary care, while providing high quality services in the community setting.

It should be noted that under the Any Qualified provider model, these will be zero value contracts with no guarantee of the volume of activity. All activity will be paid monthly on a cost per case basis, retrospectively.
